You will need to have a copy of the Request for Live Scan Form, service providers in California cannot submit your Live Scan transaction without this form. We would highly recommend that you contact your employer or requesting agency to get the correct form to avoid any errors or delays. Valid Identification needed. Call into office for more information. State and Federal Fees vary. Office Rolling fee is $30.00.

Most Common USCIS Immigration Forms
